Pesticide application records in Indiana

What 355 IAC 4-4-1 asks for on each structural pest control record, how long to keep it and what the customer must get, based on our reading of the rule. And how PestSheet covers it.

What the record must include

  • No state record requirement for general-use products in category 7a (general pest control). The full list below also applies to termite (7b), school and golf course work.

Also for restricted use pesticides (RUP)

  • Name of the responsible commercial applicator
  • License number of the responsible commercial applicator
  • Name of the supervised noncertified applicator, if any
  • Date of application
  • Start and stop times
  • Site treated
  • Principal pests to be controlled
  • Area or parts of the structure or property treated
  • Brand name of the product
  • EPA registration number
  • Amount of product used

How long to keep it

2 years. Records are made on the date of application.

What the customer gets

  • If requested, provide a copy of the records within 30 days.

Termites, fumigation and other cases

  • Schools: the school keeps the parent notification registry and gives 48-hour notice; the school keeps the record unless there is a written agreement with the company.

  • Size of the area treated

Also:

  • Reminds you to keep records at least 2 years.
  • Export the records of any period as PDF or CSV for an inspection (Pro).

The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the 355 IAC 4-4-1 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.
