What the record must include
- Business name, address and license number
- Customer name and address
- Pest(s) to be controlled
- Full product name and EPA registration number
- Quantity applied and total area treated
- Concentration (as percent active ingredient) or rate, when applicable
- Date, location, and start and end time of the application
- Expiration date of any guarantee
- Signature and certification number of the applicator
- Signature and certification number of the supervisor, when applicable
- Wind direction and speed, when applicable (not required inside an enclosed structure, for barriers within 10 feet of the building, trenching, injection or bait stations)
- A statement that the application was below label rate, when applicable
How long to keep it
3 years. Counted from the expiration date of the statement of services or contract, not from the application.
What the customer gets
- A statement of services (or contract) with the record items must be given to every customer, on paper unless the customer consents to electronic delivery, within 30 days of the service and before payment is due. An older rule says at completion of the work, so delivering at completion is the safe choice.
- When applicable, it must say: "PESTICIDE APPLIED AT LESS THAN LABEL RATE."
Termites, fumigation and other cases
- Multifamily buildings: information to the owner or manager, and to any tenant who asks.

Also:
- Reminds you to keep records at least 3 years.
- Export the records of any period as PDF or CSV for an inspection (Pro).
The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the K.S.A. 2-2455; K.A.R. 4-13-4a template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.
