What the record must include
- Date of application; if the label sets a reentry time, the hour completed
- Name and address of the customer, and the application site address if different
- Brand name of each pesticide
- EPA registration number
- Total amount of each pesticide used, with units
- Dosage or rate of application
- Names and certification or technician registration numbers of each person making or supervising the application, and names of non-certified persons involved
How long to keep it
3 years. Records must be completed in written or printable form within 24 hours of the application.
What the customer gets
- We found no requirement to give structural customers a copy of the record (the copy rule applies only to RUP in agricultural production).
Termites, fumigation and other cases
- Hypersensitivity registry: notify each registered person within 500 feet 12 to 72 hours before a commercial application; interiors of detached structures and tamper-resistant bait stations are exempt. Keep the notification record 3 years.
- Schools: no applications in common areas if students will be there within 7 hours (or the label reentry time, if longer), with exceptions such as contained baits and crack-and-crevice gel baits.

The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the 7 Pa. Code §128.35 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.
What we could not confirm
- We could not confirm whether 7 Pa. Code §128.35 was amended after 2011.
- We could not confirm the school's own notification duties under 24 P.S. §7-772.2.
