What the record must include
- Start and stop time of application
- Total amount of pesticide used
- Name and address of the company
- Name, certification number and certification expiration date of the certified applicator, and name of any non-certified applicator supervised
- Name and address of the customer
- Street address of the application (not a P.O. box)
- Date of application
- Application rate
- Dilution rate for mixing
- Total quantity of tank mix used
- Complete trade name of the product
- EPA registration number
- Adjuvants or drift agents, when the label requires specific ones
- Target pest
- Site where applied
- Size of the area treated
- Restricted Entry Interval from the label
- Copy of the product label, contracts issued and wood infestation reports issued
How long to keep it
2 years. Kept at the principal business location in Oklahoma for at least 2 years "after their date of expiration"; the safe reading is 2 years from the application or from the end of the contract, whichever is later.
What the customer gets
- Contracts are required for commercial structural applications, and they must include the information required in the application record.
Termites, fumigation and other cases
- Fumigation: all dwellings or businesses within 10 feet of the building must be notified in writing in advance.

The current version of OAC 35:30-17-21 took effect on August 11, 2024.
