What the record must include
- Product or brand name
- EPA registration number
- Date of use
- Address or location of use
- Site of the use
- Size of the area treated
- Application rate
- Dilution of the pesticide applied
- Total amount of product used
- Target pests
- Name of the person applying
- Certification number of the person applying
- Copy of any instructions given to technicians under a certified applicator's supervision
Also for restricted use pesticides (RUP)
- Time of use
- Customer name and address
- Purchase records: seller, seller address, date and quantity
How long to keep it
2 years. Counted from each use.
What the customer gets
- Public places (e.g. common areas of buildings with more than 4 dwelling units, public offices, parks): before applying, give the person who contracted a written statement with the applicator's or employer's name, address and phone; product name, EPA number, amount and target pests; adjuvants; and re-entry precautions.
Termites, fumigation and other cases
- Public places: an 8½×11 sign at each entrance, posted just before the application and for at least 24 hours. Exempt: antimicrobials, rodenticide baits in tamper-resistant stations, and ready-to-use gels, pastes and foams.
- Rented dwellings: the tenant must be notified before applying inside the unit (sign or verbal notice with date and time, product and re-entry).
- Schools: the applicator posts a sign just before applying; the school administrator notifies parents 24 hours before.

The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the 18 AAC 90.420, 90.415 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.
