Pesticide application records in Hawaii

What HAR §16-94-25.5 asks for on each structural pest control record, how long to keep it and what the customer must get, based on our reading of the rule. And how PestSheet covers it.

What the record must include

  • Date of the job
  • Client's name and address
  • Branch of pest control
  • Chemicals and treatment method(s) used

Also for restricted use pesticides (RUP)

  • Brand or common name of the product
  • EPA registration number
  • Type of formulation
  • Percent active ingredient
  • Target pest
  • Dilution rate
  • Total amount of pesticide used
  • Total area covered
  • Time and date of application
  • Address or location of the treated site
  • Certified applicator's name and certification number
  • Site treated
  • Restricted entry interval and whether posting and oral notification are required

How long to keep it

2 years.

Termites, fumigation and other cases

  • RUP: annual report to the Department of Agriculture (with tax map key number).
  • RUP may not be applied within 100 feet of a school during school hours (HRS §149A-28).

What we could not confirm

  • We could not confirm that HRS ch. 149A has no other general customer notice requirement.
