Pesticide application records in Minnesota

What Minn. Stat. §18B.37, subd. 3 asks for on each structural pest control record, how long to keep it and what the customer must get, based on our reading of the rule. And how PestSheet covers it.

What the record must include

  • Date of application
  • Target pest
  • Brand name, EPA registration number and amount used
  • Time the application was completed
  • Customer name and address
  • Applicator's name, company name, address of applicator or company, and applicator's license number
  • Any other information the commissioner requires

How long to keep it

5 years. After the date of treatment. The record must be completed no later than five days after the application.

What the customer gets

  • Immediately after the application, a copy of the record must be given to the person who ordered it if present, placed in a conspicuous spot at the site, or delivered to the person who ordered it or the site owner.
  • Rental apartments: post the label's post-application precautions in a conspicuous place inside the unit.

Termites, fumigation and other cases

  • Fumigation: also record temperature and exposure time.
  • The MDA model form also asks for temperature and wind on outdoor applications (except bait stations); the statute itself does not.

Subdivision 3 was amended in 2024.

The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the Minn. Stat. §18B.37, subd. 3 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.
