Pesticide application records in Arizona

What A.A.C. R3-8-501 asks for on each structural pest control record, how long to keep it and what the customer must get, based on our reading of the rule. And how PestSheet covers it.

What the record must include

  • Customer name and address
  • Specific site where the pesticide was applied
  • Date and time of service
  • Target pest or purpose of service
  • Trade name of the pesticide applied
  • Amount applied: % active ingredient and volume of diluted mix, or total amount of concentrate, ready-to-use product, granular material or bait stations
  • Applicator name and certification number; if uncertified, the applicator's name plus the name and certification number of the supervising applicator

Also for restricted use pesticides (RUP)

  • EPA registration number of the restricted use pesticide applied

How long to keep it

3 years. Service records with restricted use pesticides must be kept separate from other records. The applicator must give records to the business licensee within 5 business days.

What the customer gets

  • Immediately after each application, give the customer a written notice with the same information as the service record except the time (R3-8-306).
  • The notice must include, in at least 8-point type: "Warning-Pesticides can be harmful. Keep children and pets away from pesticide applications until dry, dissipated, or aerated. For more information, contact [business name and PMD license number] at [business phone]."
  • The notice may be provided electronically.
  • Services at schools must also follow the notification rules in A.R.S. §3-3606.

Termites, fumigation and other cases

  • Restricted use pesticide purchases and pesticide disposal must also be recorded (R3-8-501(C)-(D)); kept 3 years.
  • Termites/WDO: inspection reports and treatment proposals on PMD forms; a Termite Action Report Form (TARF) goes to the PMD within 30 days of pretreatment, new construction, final grade or initial corrective treatments (R3-8-503(D)).

  • Export the records of any period as PDF or CSV for an inspection (Pro).

The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the A.A.C. R3-8-501, R3-8-306 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.

What we could not confirm

  • We could not confirm the final status of the amendment to R3-8-308 and R3-8-309 (termite bait systems at final grade) noticed in July 2025; it does not change the service record or customer notice fields.
  • We could not confirm the text of the school notice requirements in A.R.S. §3-3606.
