Pesticide application records in Wisconsin

What Wis. Admin. Code ATCP 29.21 asks for on each structural pest control record, how long to keep it and what the customer must get, based on our reading of the rule. And how PestSheet covers it.

What the record must include

  • First and last name of each person applying
  • Customer name and address
  • Site treated
  • Specific description of the application location
  • Month, day, year and approximate start and end time
  • Brand, product or common chemical name and EPA registration number
  • Either concentration and total quantity, or amount per unit area and total area treated
  • Each location where the pesticide was mixed or loaded

How long to keep it

2 years. The record is completed on the day of the application.

What the customer gets

  • Within 30 days, give the customer in writing: phone number, name and license of each applicator, site and location, dates and times, product and EPA number, concentration and quantity, the label's post-application precautions for the customer, and a free copy of the label or notice that one is available.
  • Residences (including perimeter barriers): written notice to residents at the time of application, and a written offer of advance notice before the first application and every year.

Termites, fumigation and other cases

  • If the label sets a restricted entry interval, a sign matching ATCP 29 Appendix D is required at residences.
