Pesticide application records in Nevada

What NAC 555.410 asks for on each structural pest control record, how long to keep it and what the customer must get, based on our reading of the rule. And how PestSheet covers it.

What the record must include

  • Date of the treatment
  • Address where the treatment was done
  • Full name of the applicator
  • Site treated (for example kitchen, crawlspace, yard); for spot treatments, "spot treatment" plus a description of the area and spots
  • Brand or generic name of the pesticide and its EPA registration number
  • Total amount of diluted pesticide and the concentration applied

Also for restricted use pesticides (RUP)

  • Name and address of the person for whom the pesticide is applied
  • Size of the treated area
  • Total amount of concentrated and diluted pesticide applied at each location
  • Time and date of the application
  • Full name and license or certificate number of the commercial applicator, and full name of any non-certified person applying under supervision

How long to keep it

2 years. For each property treated.

Termites, fumigation and other cases

  • Preconstruction termite treatments require a copy of HUD-NPMA Form 99-B ("New Construction Subterranean Termite Service Record").
  • Fumigation records must include start and finish temperature, wind, area or volume fumigated and start and finish times.

The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the NAC 555.410, 555.700 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.

What we could not confirm

  • We could not confirm whether Nevada requires a customer copy of the record or a customer notice for general structural pest control.
  • We could not confirm the content or status of a notice of intent to amend NAC 555 published on October 17, 2025.
