What the record must include
- Name and address of the customer
- Name and certified applicator license number of the person applying
- Location of the application
- Site treated
- Day, month and year
- Starting time
- Product name and EPA registration number
- Total amount applied per application
- Size of area treated (e.g. square feet, linear feet, crack and crevice, trap or bait placement, spot treatment)
How long to keep it
3 years. Kept at the principal place of business; recorded within 48 hours of the application. Applies to all general-use structural pest control applications.
What the customer gets
- On the customer's request, provide the records within four business days.

The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the 25 Neb. Admin. Code ch. 2, §006.02 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.
