What the record must include
- Customer name and address (and who purchased the pesticide, if applicable)
- Location of the application
- Size of the area treated
- Site treated
- Pest controlled
- Brand name
- EPA registration number
- State special local need (SLN) number, if applicable
- Total amount applied per location per application
- Rate of application
- Method of application
- Date and time
- Temperature and wind direction and speed (the rule lists these with no indoor exception)
- Name and certification number of the commercial applicator
How long to keep it
2 years. The prior customer notice is also kept 2 years.
What the customer gets
- Before applying, give the customer: the pesticide to be applied, possible residue hazards, restricted entry periods, waiting periods, the application date and time, post-application label safety precautions, and any other label requirements such as posting.
Recent changes
§62-4 took effect December 18, 2024 and §28-12 took effect January 8, 2025.

The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the Wyo. Ag. Rules ch. 62 §62-4, ch. 28 §28-12 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.
What we could not confirm
- The department has proposed amendments to chapter 62 on record keeping (comments through November 24, 2026); we could not confirm the draft text.
