Pesticide application records in Maryland

What COMAR 15.05.01.12 asks for on each structural pest control record, how long to keep it and what the customer must get, based on our reading of the rule. And how PestSheet covers it.

What the record must include

  • Name of applicator
  • Date of application
  • Site treated and pest to be controlled
  • Description of the area or structure treated, with square, cubic or linear footage when the label specifies those measurements
  • Address of the treated property
  • Name of the owner or tenant
  • Common name and EPA registration number of each pesticide
  • Rate and concentration
  • Total amount of pesticide used
  • Type of equipment used
  • Time of day of application
  • Wind direction and estimated speed, except for baits in bait stations or applications in, or within 3 feet of, a structure

How long to keep it

2 years. Must be available to the department immediately on request.

What the customer gets

  • When the pesticide is applied, or when the customer signs the contract, give the customer written information: licensee name, Maryland pesticide business license number, licensee phone, Maryland Poison Center phone and the common name of the pesticide or active ingredient.
  • It must also include a copy of the product label, or of its precautionary statements section, or a precaution sheet approved in advance by the department.
  • If the customer asks, the licensee must give advance notice of an application.

Termites, fumigation and other cases

  • The pesticide-sensitive individuals list and the posting-sign rule apply to lawn/ornamental and mosquito categories, not to structural pest control.
