What the record must include
- Name of the customer
- Approximate location of the property treated
- Date and approximate time of application (ODA's guide reads this as start and end time)
- Who supplied the pesticide
- Trade name and strength of the pesticide
- Amount or concentration applied
- Specific site treated (for example wall voids, kitchen cabinets, baseboards)
- Summary of equipment used
- Names of the applicators or trainees who applied it
- Applicator's full name and license number; for apprentices or trainees, also the supervising applicator's name and license number
How long to keep it
3 years. From the date of application; available for inspection during business hours.
What the customer gets
- We found no requirement to give structural customers a copy of the record (the copy rule applies only to field crops).
How PestSheet covers it in Oregon
Before you complete a visit for a client in Oregon, PestSheet checks that these fields are filled in:
- Applicator license or certification number
- Amount applied
- Rate or dilution
- Areas or sites treated
- Application method
Also:
- Reminds you to keep records at least 3 years.
- Export the records of any period as PDF or CSV for an inspection (Pro).
The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the OAR 603-057-0130 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.
