What the record must include
- Name and license number of the certified applicator
- Application date, start time and end time
- Address or brief description of the application site
- Size of the area treated (not required for indoor spot and crack-and-crevice applications)
- Site treated
- Target pest(s)
- Complete trade name of the product
- EPA registration number
- Mixture rate, total amount of mixture used and rate of application
- Outdoor applications: air temperature, measured wind speed and wind direction (not required for general structural and termite work within 10 feet of a building)
How long to keep it
3 years. Records must be completed within 3 business days of the application.

How PestSheet covers it in Missouri
Before you complete a visit for a client in Missouri, PestSheet checks that these fields are filled in:
- Applicator license or certification number
- EPA registration number
- Amount applied
- Rate or dilution
- Target pest
- Areas or sites treated
- Size of the area treated
- End time of the service
For exterior applications, it also asks for:
- Weather (wind, temperature)
Also:
- Reminds you to keep records at least 3 years.
- Export the records of any period as PDF or CSV for an inspection (Pro).
The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the 2 CSR 70-25.120 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.
