Pesticide application records in Ohio

What Ohio Adm. Code 901:5-11-10 asks for on each structural pest control record, how long to keep it and what the customer must get, based on our reading of the rule. And how PestSheet covers it.

What the record must include

  • Recorded in English on the date of application
  • Name of the responsible commercial applicator and names of trained servicepersons applying under supervision
  • Name and address of the person contracting for service
  • Date of application
  • Pests to be controlled
  • Locations and methods of treatment for each pesticide
  • Brand name and EPA registration number
  • Concentration or dilution and total volume applied
  • Start time and completion time of the application (or when work stopped for the day)

How long to keep it

3 years. Counted from the application; kept at the business's registered location. A copy of each record must reach the registered location within 10 days.

What the customer gets

  • We found no requirement to give structural customers a copy of the record (the copy rule applies to agricultural categories).

Termites, fumigation and other cases

  • Termites: liquid treatments need a prior drawing of the structure footprint with dimensions and depth to footing; bait systems need the number of devices and activity status.
  • Schools: treat only when areas are unoccupied, or post a sign with fixed text "Pesticide Treatment Area. Do Not Enter Before..."; afterwards report date, time, area, pests, product and EPA number to the school contact.
