What the record must include
- Name and address of the customer, and application address if different
- Total square footage or size of the area treated
- Specific target sites treated
- Time and date (month, day and year) of application
- Brand name, EPA registration number and mix rate of each pesticide
- Total amount applied (diluted or ready-to-use) per location and per application
- Purpose of the application, target site and pest
- Name, business address and applicator license number of the certified applicator
- Business name and business license number (on service records and notifications)
How long to keep it
2 years. From the date of application. Records must be made within 24 hours of the application, in a uniform format.
What the customer gets
- Before each application of a restricted use pesticide with a "Danger" or "Danger-Poison" signal word, the customer must get a written statement with the business name, license number and phone, the applicator's name and license number, date and time, type of service, brand name and EPA number, and an instruction to call the business for more product information.
- The statement is left at the residence; in multi-unit housing, with the property manager, or mailed to an off-site manager at least 7 days before.
Termites, fumigation and other cases
- Termite treatments: keep a diagram of the structure with its dimensions and depth to the footer, organized separately for each structure.

The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the Utah Admin. Code R68-7-11, R68-7-16 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.
What we could not confirm
- We could not confirm on Utah's official rules site that R68-7 was not amended after September 7, 2022.
