What the record must include
- Name and address of the customer
- Location of application
- Description of the use of the area treated
- Time and date of application
- Beginning and ending time, if the application is in a school
- Brand or product name
- Estimated amount of each pesticide applied
- Target pests
- Name of the person making the application
- Trainee's name, and the supervising applicator's name and license number, if a trainee applied
Also for restricted use pesticides (RUP)
- Size of the area treated
- EPA registration number
- Total amount used
- Purpose of the application
- Anything else the label requires
How long to keep it
3 years. From the date of use or application.
Termites, fumigation and other cases
- Schools: a KDA acknowledgment form signed by the school (copy kept 36 months) and a sign at least 8.5×11 reading "Pesticide Treatment Area" and "Do Not Enter" with the re-entry time.
How PestSheet covers it in Kentucky
Before you complete a visit for a client in Kentucky, PestSheet checks that these fields are filled in:
- Amount applied
- Target pest
- Areas or sites treated
If you apply a restricted use product (RUP), it also checks:
- EPA registration number
- Size of the area treated
Also:
- Reminds you to keep records at least 3 years.
- Export the records of any period as PDF or CSV for an inspection (Pro).
The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the 302 KAR 26:030 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.
