Pesticide application records in Vermont

What CVR 20-031-012 §8.02 asks for on each structural pest control record, how long to keep it and what the customer must get, based on our reading of the rule. And how PestSheet covers it.

What the record must include

  • Name and address of the customer
  • Pest treated
  • Location of the application
  • Size of the area treated
  • Site treated
  • Time and date
  • Brand or product name
  • EPA registration number
  • Total amount applied per location per application
  • Name and certification number of the certified applicator

How long to keep it

3 years. Records are kept on a daily basis.

Termites, fumigation and other cases

  • The customer statement and fixed-text sign ("C A U T I O N / Pesticide Application / KEEP OFF UNTIL DRY") apply only to outdoor turf and ornamentals (§7.01).
  • Prior notice to tenants is the landlord's duty, not the applicator's; exempt if the tenant hired the service (§7.03).
  • Annual report including the county.

The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the CVR 20-031-012 §8.02 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.
