What the record must include
- Town and location
- Site treated
- Target pest
- Customer name and address
- For spot or crack-and-crevice work: a practical description of the extent (e.g. number of rooms treated)
- Date and time of application
- Brand name
- EPA registration number
- Active ingredient(s)
- Restricted entry interval or ventilation period, where applicable
- Method of application (type of equipment)
- Dilution agent, if other than water
- Licensed applicator's name and certification number
- General-use products: mix ratio and total mix applied
- Outdoor applications (except bait stations): wind speed and direction, air temperature and sky conditions
Also for restricted use pesticides (RUP)
- Total amount of pesticide applied (undiluted)
How long to keep it
2 years. From the date of application; records must be completed the same day as the application.
What the customer gets
- Chapter 26: give the customer a written evaluation of pest-conducive conditions.
Termites, fumigation and other cases
- Chapter 26: prior notice on the Board's form 24 hours to 7 days before applying in rental units, common areas, businesses and child care facilities (baits, gels and crack treatments in inaccessible areas are exempt, as is a resident hiring for their own unit).
- Chapter 28: outdoor signs for category 7A and a notification registry (applications within 5 feet of the structure and baits are exempt).
- Annual electronic report due January 31.
How PestSheet covers it in Maine
Before you complete a visit for a client in Maine, PestSheet checks that these fields are filled in:
- Applicator license or certification number
- EPA registration number
- Amount applied
- Rate or dilution
- Target pest
- Areas or sites treated
- Application method
- Size of the area treated
For exterior applications, it also asks for:
- Weather (wind, temperature)
Also:
- Reminds you to keep records at least 2 years.
- Export the records of any period as PDF or CSV for an inspection (Pro).
The client's name and address, the service date and start time, and the applicator's name are always recorded. The app uses the 01-026 CMR ch. 50 §1 template. It doesn't check items the app doesn't record (for example active ingredients or equipment); those remain your responsibility.
